CODE OF ETHICS & CONDUCT FOR COACHING STAFF, PLAYERS & PARENTS

Code of Ethics for Coaches & Managers
 The purpose of this code is to clarify and distinguish approved and accepted professional, ethical and moral behavior from that which is detrimental to the development of soccer within the NYSWYSA. Each coach, upon agreeing to accept a coaching or manager position will sign and abide by the Code of Ehitcs.

Player/Parent Conduct Agreement
ISC requires its players, parents and coaches to agree to abide by this Code of Conduct. All players and parents/guardians must sign this document in order to participate. The Code of Conduct includes the ZERO Tolerance Policy.

Players, parents, coaches and all event spectators must abide by the Zero Tolerance Policy.

PLAYER MEDICAL RELEASE FORM

The Medical Release form must be completed for each player and turned into the team's coach or manager in the beginning of each season.

The coach or manager is expected to have their players' medical release forms on hand and available at every practice throughout the season.


INJURY REPORTS
NYSWYSA Accident Medical Claim Form
Coaches: This form is to be kept on hand at every practice, game and ISC event. Should an injury occur, complete Section II and Section III. The form should then be given to the parent or guardian to complete and submit with NYSWYSA. 

Accident Report for Coaches
NYSWYSA requires documentation of all accidents and injuries. Coaches are required to complete this online report within 24 hours of the accident or injury, which will be kept on file for future reference should an insurance claim be submitted. Regarding Insurance Claims - All claims are handled through NYSWYSA (New York State West Youth Soccer Association). We are under their umbrella and insured through them.
